Due to growth and demand for our services, we are in need of hiring for a Admissions Coordinator that possesses strong experience in long-term care/hospital and computer skills

- Help the resident and/or their accountable parties fill out all admittance forms. Distribute to the unit and the corporate office.
- On the day of admission, enter all admissions data into the Ultracare system and give the unit a complete face sheet.
- Create fresh face sheets as necessary and promptly update the Ultracare system with any admissions, discharges, changes in contacts, insurance information, etc.
- Maintain accurate census records, placing a focus on bed availability as well as each resident's name, jurisdiction, and room number.
- Prepare a census and summary of potential NH and ALF residents, including all pertinent medical, psychosocial, and financial information of potential admissions to the institution, for review at weekly meetings of the Admissions Committee.
- Keep all admissions records up to date and compliant with current regulations for licensure, Medicare, Medicaid, and auxiliary grants.
- Send prompt email updates to departments about new admissions, hospital discharges, and internal transfers.
- The Departments of Health and Social Services may request that you establish policies pertaining to admissions procedures as well as examine and amend the ones that already exist in order to preserve regulatory compliance.
- Prepare administrative reports on admissions, marketing, and census for board meetings, meetings for quality control, and reports for jurisdictions.
- Reports for nursing home and ALF surveyors could be requested.
- May take part in admissions budget planning.
- It can be required of you to create, coordinate, and carry out an information program to market the facility in the neighborhood. This program might involve using printed materials, brochures, news releases, or public speaking events.
- To maintain regulatory compliance, update the admissions paperwork for nursing homes and ALFs.
- Plan visits to the facility for prospective residents, their families, accountable parties, social workers, and possible referral sources.
- Provide brochures, pre-admission paperwork, etc. upon request.
- Coordinate meetings between facility staff and referral sources (e.g. county social workers or hospital case
managers) as needed.
- Bachelor's with an emphasis on social work or geriatrics is preferred but not required
- High School Diploma or GED
- 1-2 years combined experience in a long-term care facility and/or as a case manager performing Level I screenings in a hospital setting, and/or experience with local Department of Social Services Long Term Care Unit.
Marketing experience is preferred
- Competitive pay of $18.42/hr- $27/hr
